Call for nomination (2 seats)

In conformity with the provisions of the Act respecting Health Services and Social Services (R.S.Q.,c.S-4.2),  notice is hereby given that 2 persons will be elected to the Board of Directors of the above mentioned  institution. Those interested in participating in the administration of Health and Social Services are invited to  declare their candidacy for the seats reserved for the population.

The polling date is scheduled for: NOVEMBER 1, 2011

To be eligible, a person must:

  1. Be a candidate only for the election in the institution indicated above;
  2. Reside in Quebec;
  3. Be of the age of majority (18 years or older);
  4. Not be under tutorship or curatorship;
  5. In the past five years not have been found guilty of a crime punishable by three years of imprisonment or more;
  6. In the past three years, not have been forfeited of office as a member of the board of directors of an institution or agency;
  7. In the past three years, not have been found guilty of an offence under the Act respecting Health Services and Social Services or the attendant regulations;
  8. Not be or have been employed by the institution or practice or have practiced their profession in the three years preceding the election date;
  9. Not have an immediate family member who is the executive director, an assistant executive director or a senior officer of the institution. Immediate family member means a person’s spouse or child, the spouse’s child, the person’s mother or father, the spouse of the person’s mother or father, or the spouse of the person’s child or of the person’s spouse’s child;
  10. Not provides goods or services for valuable consideration to the institution;
  11. Not be an employee of the Ministry of Health and Social Services, an agency, or the Régie de l’assurance maladie du Québec (RAMQ) and not receive any remuneration from the latter;
  12. Not be a member of the board of directors of an Agency or the Régie de l’assurance maladie du Québec;
  13. Not be a patient hospitalized in the institution;
  14. Have no direct or indirect relation or interest, in particular of a financial, commercial, professional or philanthropic nature, likely to interfere with the quality of the person’s decisions as regards the interests of the institution

Nomination procedure

A nomination is submitted by means of the candidate nomination form and the candidate’s declaration form obtained from the institution concerned. These documents must be signed by the candidate and the  nomination form must also be signed by two adult individuals not working for the institution nor in a center  managed by the institution and who’s main residence is located in the same area as the institution and, if  such is the case, in the same territory where the local health network is located. The original nomination  form and the candidate’s declaration must be submitted to the President of Election of the institution  concerned no later than thirty days before the date of the election, before 5:00 p.m. Documents received by  fax or other electronic means are not considered originals.
