The Medical Records Department is responsible for protecting personal information and preserving patient charts. You must be 14 years or older to access your chart or medical record.
There are two ways you can access your medical record: by mail or in person.
How to access my medical record
To access your own record or the one of your child under 14
Print and fill out the Authorization to release information contained in the medical record form, with a pen, or write on a blank page the following information:
- Your full name (or the name of your child under 14)
- Your complete address and a daytime phone number. Please indicate if a message can be left or not
- Your date of birth (or the date of birth of your child under 14)
- The full name of both your mother and father (or the full name of both parents of your child under 14)
- Please specify which documents you want (psychiatric and/or professional evaluation, hospitalization summary, etc.) and the date of these documents
- Please specify to whom the documents are to be sent. If this person is a health professional, his or her name as well as the name and complete address of the establishment where the documents are to be sent are required
To access the medical record of a deceased person
Print and fill out the Medical Record Request of a Deceased patient, with a pen, or write on a blank page the following information:
Concerning the deceased person:
- Full name
- Date of birth
- Date of death (with a death certificate)
Concerning you:
- Your full name.
- Your complete address and a daytime phone number. Please indicate if a message can be left or not.
- Your date of birth
- The reason of your demand (be precise)
- Proof of kinship
- What information you want
- Please specify to whom the documents are to be sent. If this person is a health professional, his or her name as well as the name and complete address of the establishment where the documents are to be sent are required.
All request must be signed, dated and mailed to the following address:

You can also visit the Medical Records Service in person. Please note that you won't be able to obtain the requested documents the same day.
Fees and processing time
The Loi sur les services de santé et des services sociaux (art. 24) allows 20 business days to respond to a request. An additional 10 days may be necessary to process your request.
Photocopies are free under 20; over 20, the service will charge you 38¢ per copy.
